Roots & Reach Alliance
Deeper Roots. Wider Reach. Unified Care.
The Roots and Reach Alliance (RRA) is a collaborative, multi-agency initiative designed to break down the professional "silos" that often make it difficult for people to access help. This alliance brings together medical, behavioral, housing and social service professionals to coordinate care in "real time" for our region's most vulnerable community members.
"Roots and Reach is about making sure the whole system is working for the person standing in front of us, not just the program they walked into."
Elizabeth Fernandez, MCCAC Care Coordination Manager
How It Works
The Alliance members meet weekly to collaborate with the goal of no community member falling through the cracks in our regional patchwork of supportive organizations. By sharing resources and expertise, we address the complex, intersecting needs of our clients, including:
Housing Stabilization
Behavioral Health & Substance Use Disorders
Medical Coordination
Advocacy:
By bringing a diverse network of qualified partners into a single room, the Alliance fosters vital, collaboration across our regional systems of care. We bridge the gap between agencies while strictly upholding HIPAA standards, ensuring that client privacy is never compromised in the pursuit of better support.
Our Partners in Collaboration
We are proud to stand alongside supportive organizations across five counties.
