Experiencing houselessness, facing eviction or other forms of housing instability?

Please complete each of the following 3 steps to complete your assistance request:

Step 1: Assistance Request form

Complete the Assistance Request Form in English or Spanish.

  • Please make sure you include an active phone number so we are able to contact you.

  • You can list multiple phone numbers where we are able to contact you (for example: a family member, friend, case manager).

  • If we are unable to contact you after 3 attempted calls, you will have to reapply for services.

Step 2: Release of Information

Complete our Release of Information form.

The Release of information allows our team to coordinate with partner agencies and better support you getting connected to the appropriate services.

This must be completed by the individual needing services.

Step 3: Coordinated Entry

Complete the Coordinated Entry (CE) Assessment.

CE Assessment drop-in hours:

The Gloria Center in The Dalles (2505 W 7th St)

  • Mondays: 9am - 12pm

  • Wednesdays: 1 - 4pm

  • Thursdays: 1 - 4pm

Hood River Office (606 State St, Suite 1B)

  • Mondays: 9am -12pm

If you cannot complete your CE Assessment during drop-in hours, we will contact you and complete the assessment over the phone.

This can take time, please be patient as we work hard on your application.
