Summary:

The Services Assessment and Prioritization Coordinator acts as the central coordinator for client intake, eligibility assessment, and service prioritization across MCCAC’s housing and support programs. This role ensures equitable access to services, coordinates the region’s Housing Stabilization Priority List, and facilitates both internal and external case conferencing. It requires exceptional organizational skills, deep knowledge of homeless services systems, and the ability to manage multiple priorities and projects while maintaining high-quality client and data outcomes

Key Responsibilities:

Essential Duties and Responsibilities include the following. Other duties may be assigned.

Direct Client Services (Approx. 50%)

· Conduct initial intake and eligibility screenings for clients seeking housing stabilization, shelter, or support services.

· Conduct comprehensive Coordinated Entry assessments and ensure timely placement on the Housing Stabilization Priority List.

· Provide informal diversion and resource referrals when appropriate.

· Maintain front desk coverage at the Gloria Center (5–10 hours/week), respond to client inquiries, and coordinate scheduling to prevent service bottlenecks.

· Translate and assist during client appointments as needed, especially for Spanish-speaking clients.

Program & Partner Coordination (Approx. 35%)

· Lead and facilitate bi-monthly or more frequent case conferencing meetings with MCCAC staff and external partners.

· Collaborate with MCCAC caseworkers and program managers to ensure client follow-through and referral placement.

· Serve as the primary point of contact for digital referral platforms and coordinated entry workflows.

· Lead community-facing projects (e.g., 100-Day Challenge, Coordinated Entry Trainings).

· Contribute to hiring panel interviews and team onboarding for related staff roles.

Data and System Workflow Management (Approx. 15%)

· Ensure timely and accurate data entry in the Homelessness Management Information System (HMIS) and other digital referral platforms.

· Design, document, and train staff and partners on coordinated system workflows.

· Support data quality initiatives and reporting needs in collaboration with leadership.

 

Education & Experience:

  • High school diploma or GED required; degree in Public Administration, Social Work, Data Science, or related field preferred.

  • At least 2 years of experience working in homeless or social service programs

Hourly and Benefits Information

Hourly: $25.75

  • Benefits: MCCAC offers an excellent benefits package including Blue Cross medical insurance with the agency covering 90% for employees and dependents. Dental and vision coverage. Life insurance, 401k, paid vacation and sick leave.
